As an alternative to this online withdrawal form, a sample withdrawal form (PDF, 46kb) is also available for download; however, you are not obliged to use this. A statement to the same effect is sufficient.
If you use this online withdrawal form and submit your details to us via the “Confirm withdrawal” button, we will immediately send you a confirmation of receipt containing information on the content of the withdrawal notice, as well as the date and time of its receipt, to the email address provided in the online withdrawal form.
